Not the run ---- not the stolen base (if one is stolen) --- not the applause. The original hitter is replaced because he is normally slower and would have been a liability on the base path. The “courtesy runner” rule and phrase was probably devised and coined by the parent of a slow running catcher that used the excuse of “he needs to put on his catching equipment” whenever his son was replaced by a pinch (courtesy) runner. In college the “courtesy runner” provision didn’t exist so whenever the pinch runner entered the game the hitter’s day was done.

Not always the case -- on my son's high school team, the pitchers and catcher are always replaced by a pinch runner even though they run fast. The coach does this to give opportunities to those who are not starting in the game.
