If starter A is subbed with player B, can starter A be subbed for a different player other than B? Upon A returning to the game, can both players be on the field at the same time? Or B must be removed when player A reenters?

I assume you mean NFHS.

A players can only ever be in in the same spot in the batting order. When B subs for A, he is now in that spot. B occupies that spot until he is replaced. A can only reenter in that spot. Thus, A and B can never be in the lineup at the same time.
