Tell him to hit the front of the ball. Smile I teach to hit the front of the ball, through the back of the ball. I want acceleration through the ball not to the ball. This is a lot like telling a pitcher to throw through the catcher.

Not a big believer in hitting the top of the ball. My guess is he is collapsing the backside trying to hit hard. Tell him to keep his posture and hit either fast or quick (not hard).

I did this with no video.
JPII,
While his swingplane may off, a more likely culprit is timing. I would check where he is making contact because pop ups and weak grounders are indicative of the swing being late. I would bet most of his popups and grounders are middle of the field to right field/1st base; rarely one down the 3rd base line if ever. Even with a good swingplane, if the swing is late contact is being made at the wrong point of the swing...instead of off his stride foot he is making contact in front of his cup (trying to get reference points). Assuming his swing is on a slightly upward plane, he would be making contact as before the pat gets on plane (bat would be naturally below the plane of the pitch as contact is made too deep in the swing).

Confused?? Having a tough time putting this into words.
Lots of possibilities here, as stated earlier, "hard to say without video."

If there is a flaw in the bat path, seems to me that he would have problems centering the ball up off the tee also? If that's the case then some major "retooling" is necessary.

When I see a hitter, in BP or games doing what you are describing, I look for the obvious 1st. Is he really "concentrating" on picking up the center of the baseball at release (that takes some reflection by the hitter to answer truthfully)?? The next thing I look for is if they are "pulling off"(head, front shoulder). That causes the bat to drop, as the back knee collapses resulting in a ball they will either top (weak ground ball) or miss low (weak pop up).

When PrePrime was little, I wrote his initials between the "two seams" on all are hitting balls. On the tee, I placed the ball with the initials facing him, so he always had a target "to look for" in the center of the ball. Ten years later he doesn't just see a baseball, he's always looking for the center.
