Tryout rating forms?

Hi, just wondering if anyone new where I could get rating forms for a tryout I'll be helping to run. I'll be looking for throwing ability, hitting, fielding and running speed.
Like most tryouts some people already have there opinions set so I'm trying to help some of the lesser known kids show there stuff. If it's on paper it's hard to argue with, any help would be appreciated. I guess I could make my own but was hoping there might be something already out there.
