Son has UCL sprain. Has seen 2 ortho docs so far - both have worked with MLB teams. One says - rest x 2 weeks then start slow throw. Other one wants additional test but says most likely needs TJ surgery. How many more opinions do we get ? ( sons has rested 2 weeks and still having pain)

Of all the dozens of pitchers I know that have dealt with this issue I don’t know one that didn’t eventually have TJ surgery. If they wanted to keep playing. IMO UCL injuries just don’t heal any other way. Even a “sprain” is damage and further throwing once it’s detected always seems to lead to more damage until surgery is the only alternative.

What grade is the sprain?
Possibly true that your son may eventually need TJS, but most doctors don't rush to operate after 2 weeks of rest. He should be in PT and may have to be for awhile.
IMO you need a better opinion.
I don't know where you are in the north but I suggest a visit to Dr David Altchek in NYC. He is the Mets doctor and operated on my son a few times. His diagnosis and advice is spot on.
I’ve come to believe that most kids who throw hard will need it, it’s just when. Timing is everything. I think you need another opinion, but if one doctor says he needs it, it’s probably at least questionable that he doesn’t. He could possibly do an internal brace. Is he a junior in eligibility?
