I suggest you send this profile info with your initial contact letter. Make the cover letter "personal" for this coach and program. Keep it short, 1 page is best with attachments. If you have a game schedule, send it. If you have plans to attend showcases or a major tournament later in the season, include this info with dates in your cover letter. Would generally second those suggestions.

We had great results with a clear cover letter directed to a specific coach outlining concisely why you feel that you are a good fit for that particular program.

Then a resume with a picture, and listing staistics, testimonials and trusted references who the coach can call with confidence.
