Just glanced at an ASMI study.

Although number of pitches, velocity and some other factors had fairly significant impacts increasing the likelihood of serious injury from 4 to 2.5 times the number one culprit by a large margin was throwing regularly with a tired arm.

Pitchers who threw regularly with a fatigued arm were 36 times more likely to need surgery.

That study was for adolescent pitchers.

Their study on youth pitchers had this conclusion:

"No relationship between poor mechanics and increased risk of pain could be established."
